SELF-SCHEDULE YOUR HEALTH SCREENING

PLEASE NOTE: Your Employee Health appointment must be completed at least Five to Seven BUSINESS DAYS after your offer. If your clearance is not processed in the required time frame, your start date will be delayed. Please contact your Onboarding Coordinator or Human Resources with any questions. 

Please ensure the name you make your appointment with on Full Slate and the name you have on your ID are the same at the time of your appointment. Bring your Photo ID and completed Health Packet from your New Hire Portal with you.   If you are 15 minutes late, you will be rescheduled. 

Please also bring your identification for your I-9 (usually a passport or a driver's license and social security card) to HR before or after your employee health visit
